#83: Work Life Integration| Trauma Surgeon And Mother Of Six | Qaali Hussein MD

The term moral injury has been gaining popularity in the internet-sphere, especially after, Wendy Dean MD and Simon Talbot MD's article was released on STAT news. Moral Injury is an important topic to discuss as this terminology offers a broader understanding than the word "burnout." Burnout is often thought of in a way that health professionals have something wrong with them: they aren't resilient enough, they need to learn to meditate or do yoga, for example. Yet moral injury suggests something larger is at play. The consequences of this terminology and mindset change are immense, as we learn that hospital dynamics, insurance, litigation, electronic medical records, and policy must evolve, for health professionals to thrive. Learn from our guests Wendy Dean MD and Simon Talbot as we discuss moral injury and it's implications on healthcare. Our Guests Wendy Dean MD "A psychiatrist by training, Dr. Dean left clinical medicine when generating revenue crowded out the patient-centered priorities in her practice.  Her focus since has been on finding innovative ways to make medicine better for both patients and physicians--technologically, ethically, and systemically. Essentially, we must decide to accept all the parts of our current life including our physical, emotional, spiritual, mental, environmental, financial, and relationship situation. Everything around us is a reflection of us. How do we change the parts we don't love? We first start with acceptance and work through our feelings of guilt, shame, and fear that come into our lives. As we accept all that has happened, we can start to look in the present and ask ourselves how we can move forward to shift our future. It's not selfish to take care of yourself. Wise-selfishness is a concept I learned from  The Book Of Joy by the Dalai Lama and Desmond Tutu. It is the simple idea that in service of others, it’s okay to serve yourself. You may have heard of the golden rule: “do unto others as you would do to yourself.” But did you know that the golden rule is bi-directional? Do unto yourself as you would for others.   Being “selfish” is a great way to help others. When we utilize positive self-care we become the example. When we have great energy and feel refreshed we can better care for others. When we remove the guilt and shame around taking care of ourselves, we can better serve. Utilizing Locum Tenens, Dr Evans has the freedom to be a part-time physician at small clinics and hospitals around the country allowing her to, further gain exposure in a variety of hospital cultures, and hone her procedural skills . Oh yeah, absolutely, I can look back at my own training, but also, in talking to people going through training now, it's true. You come in as one kind of person at the beginning of your training... you know you're first day, the day you got accepted and we have this wonderful talk telling you you're going to have this huge future in front of you, and then over time, your viewpoint (as the work piles on and the stress piles on and the sleep disappears), you find your perspective shrinking and you lose it. I think a lot of people that I trained with, and I hear it from young people today, lose some of the compassion, enthusiasm and wonder of medicine as you go through the actual process of becoming a physician. I know you started The Happy Doc because that's one of your concerns that there are so many physicians who are not as happy as they thought they would be, and so I'm hopeful that doing things like writing and having other aspects of your life that bring you joy, that it can start to bring that joy back into the practice of medicine as well. Dr. Salber reflecting on her choice in medicine You know what, I still love it, and when I have people talk to me and they say, "oh I don't think I want my kid to go to medical school," I say first of all, it's not really your choice you know (by the time your kid's ready to go to medical school they're young adults), they get to choose what they want to do. Second, its such a wonderful field. Because even if you choose not to practice full time, or if you choose to practice part of your career but not all of your career, you still take that incredible knowledge and training and you can apply it to all sorts of other things, public health for example, or the business in medicine like I did, or writing, or a whole host of other things. You can use that training and continue to learn and explore.  Look at the bigger picture, you career can move and change That's the beauty of medicine, you don't have to do the same thing for your entire career, you can take that knowledge, experience, and credentials and try a bunch of different things, if that's the kind of lifestyle you want to have. It's  probably easier for me to say that from my end of the age spectrum, than when you're in the middle -- I do remember how all consuming it was in the middle of training. It was really hard to think about, your life after training. But it does exist, just like life after family does exist, for example. It doesn't mean that the family isn't still important to you, but you're not wrapped up in "oh i gotta get to the soccer game" or all the kinds of things that make it tough to balance family, life, and work during the years when the kids are young. Oh yeah, absolutely, I can look back at my own training, but also, in talking to people going through training now, it's true. You come in as one kind of person at the beginning of your training... you know you're first day, the day you got accepted and we have this wonderful talk telling you you're going to have this huge future in front of you, and then over time, your viewpoint (as the work piles on and the stress piles on and the sleep disappears), you find your perspective shrinking and you lose it. I think a lot of people that I trained with, and I hear it from young people today, lose some of the compassion, enthusiasm and wonder of medicine as you go through the actual process of becoming a physician. I know you started The Happy Doc because that's one of your concerns that there are so many physicians who are not as happy as they thought they would be, and so I'm hopeful that doing things like writing and having other aspects of your life that bring you joy, that it can start to bring that joy back into the practice of medicine as well. Dr. Salber reflecting on her choice in medicine You know what, I still love it, and when I have people talk to me and they say, "oh I don't think I want my kid to go to medical school," I say first of all, it's not really your choice you know (by the time your kid's ready to go to medical school they're young adults), they get to choose what they want to do. Second, its such a wonderful field. Because even if you choose not to practice full time, or if you choose to practice part of your career but not all of your career, you still take that incredible knowledge and training and you can apply it to all sorts of other things, public health for example, or the business in medicine like I did, or writing, or a whole host of other things. You can use that training and continue to learn and explore.  Look at the bigger picture, you career can move and change That's the beauty of medicine, you don't have to do the same thing for your entire career, you can take that knowledge, experience, and credentials and try a bunch of different things, if that's the kind of lifestyle you want to have. It's  probably easier for me to say that from my end of the age spectrum, than when you're in the middle -- I do remember how all consuming it was in the middle of training. It was really hard to think about, your life after training. But it does exist, just like life after family does exist, for example. It doesn't mean that the family isn't still important to you, but you're not wrapped up in "oh i gotta get to the soccer game" or all the kinds of things that make it tough to balance family, life, and work during the years when the kids are young. When you hear the words Muslim, female, and mother of six, the image of a trauma surgeon probably is not the first thing that comes to mind, but this next guest is breaking all the stereotypes! This week, we have the absolute pleasure of learning from the amazing Qaali Hussein, MD, a Muslim mother of six who has overcome all the challenges that have stood in her way.  [vcex_spacing size="10px"] Learn about the inspirational and amazing story of a child caught in a Civil War in Somalia, who got her first glimpses of trauma trying to protect her grandmother. Learn about the resident who was told she couldn't be a mother in residency, but did so anyway. A glimpse into the Conversation: Why Trauma: In Somalia when we were leaving, there was a Civil War. On our way out, we had a trauma occur within our family. I was in a car with my grandmother, we got shot at. My grandmother ended up getting shot in the legs and the bullet went through both of them. We had to get off the road to help control and stop the bleeding. At the age of seven, as I was putting my hands on her bloody legs, that was my first exposure to trauma. [vcex_spacing size="10px"] Throughout my studying and training I never planned on doing trauma, but when I started doing it, it was an automatic connection. I don't know if it was from that initial exposure to experiencing trauma, but it was something that automatically made me feel, you know, someone is hurt and you can do something. You can do something significant to save their lives. That's been one of the things that pushed me towards trauma. It was just an instant affinity as soon as I started doing it. Following The Plan: I've always wanted to say my plan was to be a doctor. Plan "A" was to be a doctor, plan "B" was to be a doctor, and plan "C" was to be a doctor. Plan D was to stay at home and have a bunch of babies.  How do you know you chose the right path: [There] are days that are absolutely exciting and even if [you've] been up for 24 hours, 30 hours straight, by the end of it is very exhilarating. If you have a passion for something like that, I don't think any limitations should exist with your pursuit!     [vcex_spacing size="10px"] The Struggles of appearances, and overcoming them: I've literally been called a terrorist by a patient of mine. I've been given religious lectures about which book is the true book, and you know, those things happen. But the good thing is, I take care of patients that are very very sick. So when they see that I'm trying to help them, all those things fall by the wayside and we don’t have to talk about that. In this role she is an advocacy coach and resident team leader for Medical Student and Resident wellness.  Not only is Dr. Dakis an influence in the field of medicine and the wellness of resident physicians and their medical student predecessors, she is quite the musician. I am so at peace with everything I am doing, there is no price tag for that. Our next guest Dr. Lisa Splittstoesser is an Internal Medicine Specialist  on the island of Kauai in Hawaii. She utilizes integrative, lifestyle-based medicine in her clinical care. She owns her own clinic and is truly happy, being able to work in a way that is congruent with her values as a doctor, and a human being. She sees patients for up to an hour-and-a-half and much of her work time is spent  with complete face-time with the patient. She has created a welcoming environment for her and her patients to not only get treated, but even more importantly, to heal. I think medicine is like us, young people with a lot of diversity, diverse interests and more emphasis on work-life balance. We have our own lives, and like to take those experiences, and integrate them into how we take care of patients.   Journey to Emergency Medicine and brand new boots: I was planning on doing law and policy, but I realized I had a fundamental gap of understanding of what it was like to be inside healthcare. I went on a sort of reconnaissance mission with full plans of being a consultant. Then I went to the Emergency Room and I had on these brand new spanking boots. I turned around and a trauma came in, they put in a chest tube and blood spattered all over my brand new boots... and I was like, "this is soooo cool!"   Please listen to Amy's memorable/funny story about 7 minutes into the conversation, it's fantastic...   Describing medicine to a young student: You are going into one of the most personally gratifying industries out there. You do have the potential to actually save lives and have huge impact. I think you will need to understand the obvious sacrifices. There's emotional tolls you must be ready for. You are going to have patient complaints, you are going to have people pass away, you are going to have people that you advise into "do not resuscitates". That's an incredible impact, and incredible weight to take on, and you realize you are making life and death decisions for people with 3 times the life span that you do. You have to be really confident in your training and in your intention to feel comfortable about making those conversations and decisions.   Tip for keeping up with life as a doctor - Triage your life/worst-case scenario: We are into triage in Emergency Medicine. There's things that are really urgent and high risk. There's thing that are urgent but lower-risk. I think if you triage appropriately, you will get away with time-management in a way that makes you happy. In triaging ask yourself: what's the worst thing that can happen. If I miss a deadline for submitting this paper [for example] - well I won't get published. That's not a negative that's a neutral, I'm just not published.   Attitude, the key to medicine: I think that attitude is ultimately the key in medicine. I think you also need an internal gratitude for life in general. You realize life is fragile, relationships are fragile, you see incredible people... like ninety year-olds that have been married for seventy years and die within one day of each other... that's actually really beautiful as well as tragic. I think we have to recognize the silver-lining to those sort of situations, because we are so privileged to be a part of those in so many people's lives. I think if you come in with a bad attitude, no matter what specialty or industry... you are never going to ever be happy. You have to see the good in what you are doing, and accept the frustrations or get excited about how to change them.
